For the past few years, we have been recommending that patients with persistently elevated serum PSA levels should consider being evaluated with MRI of the prostate, before going for prostate biopsy.

A recent comprehensive review of the research supports this approach. In this review, it was found that MRI-directed evaluation of the prostate was superior to systematic prostate biopsy in making a correct diagnosis of clinically important prostate cancer,  reducing the number of unnecessary biopsies and reducing the detection of unimportant cancers.
